For Chiropractic treatment a referral is not required. 

For physical therapy treatment a person can receive PT for 21 days or 8 treatments (whichever comes first) without requiring a referral from a medical provider.

For injury prevention, wellness, performance based therapy a referral is not required. 

Yes we are authorize to accept FSA and HSA as form of payment and can provide an itemized receipt for submission if needed. 

Unfortunately we cannot accept care credit at this time.
